Dysmenorrhea (menstrual pain) is caused by neurogenic inflammation that occurs in the vascular circuit of uterine
artery and is triggered by activation of mast cells. This inflammation causes excessive introduction into uterine
bloodstream of neuromediators with a vasodilator effect (histamine, leukotrienes, prostaglandins). This is why
violent menstrual pain is triggered, which also causes alterations in start dates and length of cycle.
The mechanism of action is the same as in headaches, but location changes. In fact, in headaches the cause is a
narrowing of meningeal vessels, in dysmenorrhoea contracture is borne by uterine arteries.
Menopausal Hot flashes are caused by a neurogenic inflammation of utero-ovarian nerves with diffuse vasodilation
caused by a massive and chaotic release from mast cells and introduction into uterine bloodstream of neuromediators
with a vasodilator effect such as CGRP (calcitonin gene-related peptide). This excess acts on thermoregulatory center
of hypothalamus and causes peripheral vasodilation, generating flushing. There is also free radical damage.
In the Endometriosis there is an excessive release of inflammatory mediators like histamine, substance P, serotonin
and neurotrophic substances, like NGF (nerve growth factor). Inflammatory mediators cause oxidative damage
from free radicals, while NGF causes increase of nerve endings resulting in hypersensitivity to painful stimuli and a
burning perception to tactile stimuli. All these neuromediators are exaggerately emitted by mast-cells due to a
dysregulation of their metabolism.
